Healthcare in Alicante
Health and medicine in Alicante: how insurance works, where to go and how to find a Russian-speaking doctor.
Healthcare in Alicante: how the system works and what to know before you move
Healthcare is one of the first practical questions that arises right after you move. In Alicante, the healthcare system has two tiers: public and private. Access to each depends on your residency status, whether you have insurance, and your knowledge of the language. It is better to understand this in advance — otherwise your first doctor’s visit will turn into a stressful ordeal.

Public healthcare: how to get access
Spain’s public healthcare system (Sistema Nacional de Salud) is considered one of the strongest in Europe. In Alicante it is administered through the Generalitat Valenciana. The key to the system is the tarjeta sanitaria individual (TSI, health card).
How to get a TSI:
- First, register with the municipal register — the padrón municipal. This can be done at any local Ayuntamiento (town hall) office.
- With a confirmed padrón, passport, and proof of status (residence permit, NIE, or refugee status), visit your nearest centro de salud (local GP clinic).
- Timeline: the TSI is issued within a few weeks of submitting the application; in some cases, sooner.
Once you have the card, you have access to free consultations with your GP (médico de cabecera), some lab tests, specialist referrals, and emergency care. Specialists are only accessible by referral, and waiting times can be 2–6 weeks or more, especially for planned procedures.
Main public hospitals in Alicante:
- Hospital General Universitario de Alicante — the city’s largest facility, accepting emergency patients 24/7.
- Hospital Universitario de Sant Joan d'Alacant — the second largest, covering the south of the metropolitan area.
Insider tip: Alicante’s public healthcare system is overstretched. In summer this is especially true, as the province’s population doubles with tourists. Routine appointments are effectively frozen in July and August.
Private clinics in Alicante: costs and options
For most newcomers, particularly in the early period without TSI, private healthcare is the primary option. A GP consultation at a private clinic costs 40–80 €, a specialist visit 80–150 €, depending on the field. Basic blood tests start from 30–60 €.
Well-known private networks in Alicante:
- Clínica Vistahermosa (HLA group) — one of the largest in the province, with a broad range of specialists and modern equipment.
- Hospital Quirónsalud Torrevieja — conveniently located for residents in the southern part of the province.
- Policlínica San Carlos — a multi-specialty center in the central part of the city.
Consultations are generally conducted in Spanish. English is available with some doctors, particularly at clinics catering to expats. A Russian-speaking doctor in Alicante is a rarity: a handful of practicing specialists exist, but appointments are typically arranged through word of mouth or Russian-language groups. More common is the format of a medical interpreter — someone who accompanies you to appointments and translates; the service costs 30–60 € per visit.

Health insurance in Alicante: what to choose and how much it costs
Private health insurance covers most situations without the queues of the public system. For a Non-Lucrative Visa (residence permit without the right to work), proof of insurance is a mandatory requirement when submitting your application.
| Insurer | Best for | Price (per month) | Note |
|---|---|---|---|
| Adeslas | Families, long-term residents | from 60–90 € | Wide network of doctors in Alicante, good specialist choice |
| Sanitas | Expats, digital nomads | from 70–100 € | App and online consultations, some English-speaking doctors |
| Asisa | Retirees, older clients | from 80–120 € | Good chronic-illness coverage, dentistry as an add-on |
| DKV | Younger, health-conscious users | from 55–85 € | Affordable entry point, but narrower network outside major cities |
| Mapfre Salud | Families with children | from 65–95 € | Pediatrics included in the basic policy |
Prices in the table are market reference points: government fees and third-party rates at the time of publication. These are not SVOY prices — our service fees are provided on request and in the ’Services’ section.
Key insight: the cost of a policy depends on your age at the time of first enrolment. Premiums rise sharply after 45. If you are planning to relocate, it is worth taking out insurance sooner rather than later. In addition, most policies include a waiting period (período de carencia) for certain services — ranging from 3 to 12 months after signing. Maternity, oncology, and surgery often carry an extended waiting period.
Dental care and emergency services: a separate conversation
Dental care is almost entirely absent from the public system — only emergency extractions are covered. Everything else is private. Private dental clinics in Alicante are affordable: appointment from €50–80, implant — €1,200–2,000, braces — from €2,500.
Emergency services (112) are available to everyone without exception and are free in genuine life-threatening situations. A call for a non-life-threatening condition may result in a bill if you have no TSI or insurance.
Common mistakes
- Not registering for padrón right away. Without municipal registration, the TSI is unavailable — and people spend months paying for private appointments when they were already entitled to public healthcare.
- Using travel insurance for a residence permit application. A travel policy is not accepted when applying for a Non-Lucrative Visa — you need full medical insurance with no coverage limits.
- Ignoring the waiting period. People sign up for insurance expecting to use it immediately — without reading the carencia conditions for specific types of services.
- Searching for a Russian-speaking doctor and losing time. Russian-speaking specialists are extremely rare; the better strategy is to find a medical mediator and choose a doctor by competence, not by language.
- Calling an ambulance for non-urgent conditions. This overloads the system, creates waits of up to 4–6 hours in the emergency room, and can result in a bill for those without coverage.
- Not checking the clinic network before buying a policy. The insurer may be well known, but the specific specialist you need may not be in their network in Alicante. Always verify the cuadro médico list for the province of Alicante before signing any agreement.
Bottom line: what to choose and for whom
If you have just arrived and have not yet received your TSI — get a private policy immediately: Adeslas or Sanitas cover most situations and are well represented in Alicante. At the same time, register on the padrón — this opens access to the public system and reduces long-term costs. For families with children and people over 50, private insurance remains a priority even after receiving a TSI: waiting times for scheduled care in the public system are simply too long for everyday life.
